How to Register as a New Patient
If you would like to register with the surgery you will need to complete the appropriate forms for each person and hand them in to reception. It is important that these are handed in personally so that we discuss any queries with you. If you are receiving any repeat medications you will need to book an appointment with a GP so that he/she can add these details to your screen. In any case it is advised that you book for a health check with one of the Nursing Team so that we can accurately measure your height, weight and blood pressure reading etc. We also ask you to provide a urine sample so that we can perform routine tests on this.
Each adult will need to complete:
- GMS1 Form
- Adult Patient Health form
- Ethnicity Questionnaire
- Summary Care Agreement Form
- GP Online Services Form

Each Child under 16 will need to complete:
A Summary Care Record will be created for each child unless the parent/guardian completes a form for them to opt out of this.
PLEASE COMPLETE ALL THESE FORMS AS FULLY AND ACCURATELY AS POSSIBLE.
