Our community Nursing Team travels to the patients own home and local residential homes, assessing the nursing needs and providing care to house bound patients who are unable to travel to the surgery.
Both Tavyside and Lifton referred patients can contact the community nurses by telephone on 01822 617602. There is an answerphone which is checked regularly throughout the day if they are unable to answer your call.
The Health Visitor
The Health Visitor Team are based at Tavistock Children's Centre and can be contacted on 0333 2341901.
Health Visitors are qualified nurses with further specialist training, who are experienced in child health surveillance, health promotion and health education. They work with the families of the 0-5 age group across the surgery population. Health Visitors work in conjunction with the School Nurses who can also be contacted on the same number.
The Midwifery Team are based at Tavyside Health Centre and can be contacted on 01822 619711 (Tavistock). They work alongside the doctors and Health Visitor.
The GP can refer you to a local podiatry service. Where you are seen depends on where you live. Joanne Whiteley also carries out a private clinic at Tavyside Health Centre on a weekly basis. If anyone wishes to use this service you can obtain her details at the reception desk.
If you need to see a physiotherapist, your GP will refer you. Most of are patients are seen via Tavistock Hospital but you can choose Derriford Hospital if this is more convenient for you. One of the Team will contact you directly to make an appointment.
Alternatively you can now self-refer. Your GP can supply the necessary form at your consultation. You can also ask for a form at the reception desk or click here to download a form.
The surgery hosts a counselling service which can be accessed by referral from your GP. The counsellor is registered by UKRC and is accredited by the British Association of Counselling and Psychotherapy. They work to the BACP ethical framework for good practice.
Depression and Anxiety Service
The Depression and Anxiety Service, now called Talk Walks, is a service for people in the South and West of Devon who are 18 years and over who are feeling stressed, anxious, low in mood or depressed. You can self refer to this service simply by calling 01626 203500
St Luke's Specialist Palliative Care Nurses
Derek Hart is our Specialist Palliative Care Nurse. He gives support and comfort to terminally ill patients and their carers. He is in regular contact with St Luke's Hospice and can advise doctors and patients alike on advances in care of the terminally ill. Referrals to Consultant via DRSS
If during your consultation with the GP it is suggested that you need to be referred to see a Consultant for further investigations, this would generally be done through DRSS. This is a very valuable service which offers patients much more choice and flexibility.
Once your Doctor has decided a referral is required, you will be sent an information leaflet which will have all the details you need concerning your referral. you will then need to contact the team who will assist you with your booking. They will discuss all options available to you.
All referrals require blood pressure, height, weight and smoking status, taken within the last 12 months. We have a specialist machine in our waiting room (Tavistock), which can undertake these measurements for you.
Admiral Dementia Nurse
Mary-Jo is West Devon Primary Care Networks Admiral Dementia Nurse. She works with local Surgeries including Abbey Surgery, Bere Alston, Lifton Surgery, Tavyside Health Centre, Yelverton and Princetown Surgeries.
